What did we do?<\/strong><\/p>\n\n\n\n<p class=\"has-black-color has-cyan-bluish-gray-background-color has-text-color has-background wp-block-paragraph\">We know that trans men and transmasculine people face health challenges and barriers to health care, but there has been little research on transmasculine health in India. Research data are important for bringing attention to issues faced by communities to support advocacy, services, and funding. To fill this gap, a group of trans activists and researchers (both trans and cisgender) from India, Canada, and the Unites States began to work together in 2017 to develop community-based research on transmasculine health in India. As a first step, we applied for a small research planning grant from the Canadian Institutes of Health Research, which we used to hold consultations with trans men and transmasculine people in three cities.<\/p>\n\n\n\n<p class=\"has-black-color has-cyan-bluish-gray-background-color has-text-color has-background wp-block-paragraph\">The consultations were organized and co-facilitated by community leaders from TWEET Foundation, Sampoorna Working Group, and Aneka Trust. Consultation participants were asked for their perspectives on the most pressing health and human rights issues facing trans men and transmasculine people, and to identify their priorities for future research. Each group discussion lasted 2-3 hours and participants were provided refreshments and an honorarium. Participants did not need to provide their names or any other information that could personally identify them. The consultations were reviewed and approved by research ethics boards at the Centre for Sexuality and Health Research and Policy (C-SHaRP) in Chennai and Unity Health Toronto in Toronto, Canada.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<p class=\"has-black-color has-text-color has-background wp-block-paragraph\" style=\"background-color:#959595;font-size:16px\"><strong>Who did we speak to?<\/strong><\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-background wp-block-paragraph\" style=\"background-color:#959595\">Consultations were held from April to October 2019. There were 11 participants in Delhi, 12 in Mumbai, and 8 in Bangalore. Participants ranged in age from 20 to 46 (average=29) and self-identified as trans men (55%), male (42%), genderqueer (3%) and\/or trans masculine (3%). Educational backgrounds ranged from lower primary to postgraduate education but more than half of participants (66%) had completed university education.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<p class=\"has-black-color has-text-color wp-block-paragraph\" style=\"font-size:16px\"><strong>What did they tell us?<\/strong><\/p>\n\n\n\n<div class=\"wp-block-coblocks-accordion\">\n<div class=\"wp-block-coblocks-accordion-item\"><details><summary class=\"wp-block-coblocks-accordion-item__title has-background has-light-green-cyan-background-color has-text-color has-black-color\">Healthcare<\/summary><div class=\"wp-block-coblocks-accordion-item__content\" style=\"border-color:#7bdcb5\">\n<ul class=\"wp-block-list\">\n<li><strong>Access to healthcare <\/strong>was a big issue for the community. Participants cited a variety of issues leading to difficulty receiving the care that they needed, including:<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Lack of information <\/strong>in the community about how to find providers and what kind of care they needed<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><meta charset=\"utf-8\"><meta charset=\"utf-8\"><em>\u201cWe don&#8217;t have any awareness regarding the hormones, neither the surgeries, and \u2026 the things that you&#8217;re supposed to know while taking the HRT [hormone replacement therapy], and what are the side effects and how we\u2019re supposed to take care of our health. So we have zero knowledge about it.\u201d <\/em>(Bengaluru)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>&nbsp;<strong>Lack of available providers<\/strong>, especially in rural areas<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cPeople migrate to metro cities. Metro cities are a bit supportive because the doctors are able to practice more regarding all these things\u2026But there is nothing for the people in villages.\u201d <\/em>(Mumbai)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Dealing with uninformed or <strong>misinformed providers<\/strong>, whom participants needed to educate<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cHe will never say \u2018I don\u2019t know\u2019. That is a doctor\u2019s attitude.\u201d <\/em>(Bengaluru)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Being treated poorly by healthcare providers or <strong>being denied care<\/strong><\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cOne of my friends who, a trans man\u2026asked the doctor \u2026 How are you going to do the surgery? Because he wanted to know what his chest would look like and he needed the information. So the doctor screamed at him, saying \u2018Are you the doctor or am I the doctor? I\u2019ll decide how it should be, it is none of your business. Just shut up and go do the surgery.\u2019 That is the attitude\u201d <\/em>(Bengaluru)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Low standards of care<\/strong>, such as poor surgery outcomes or improper hormone injection techniques<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cIn fact, we are like guinea pigs to the doctors. We\u2019re guinea pigs and whatever they want to learn, experiment, they get free bodies.\u201d <\/em>(Bengaluru)<\/p>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cHe went to his chemist [to receive his hormone injection]. He kept on looking at it, [wondering] \u2018is he doing it right, is he not doing it right?\u2019 So the chemist said \u2018if you\u2019re so concerned, just learn it and do it yourself. And he said \u2018okay, I can do it\u2019. So he got his first injection wrong and he was in pain and he couldn&#8217;t get up for four days.\u201d <\/em>(Delhi)<\/p>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cThey want to experiment doing the surgery. They get a chance to do it, they&#8217;re doing it\u2026So they don&#8217;t want to miss that opportunity of experiment, so that\u2019s why they do it in that rubbish manner.\u201d <\/em>(Bengaluru)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Difficulty navigating the<strong> complicated GID certificate process<\/strong> and all the necessary referrals<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>High cost <\/strong>of gender affirming procedures and lack of insurance coverage<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n<\/div>\n\n\n\n<div class=\"wp-block-coblocks-accordion\">\n<div class=\"wp-block-coblocks-accordion-item\"><details><summary class=\"wp-block-coblocks-accordion-item__title has-background has-pale-cyan-blue-background-color has-text-color has-black-color\">Mental Health<\/summary><div class=\"wp-block-coblocks-accordion-item__content\" style=\"border-color:#8ed1fc\">\n<ul class=\"wp-block-list\">\n<li>Participants talked about significant <strong>mental health challenges <\/strong>faced by the community, resulting from discrimination, social and family pressures, and difficulty accessing healthcare.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cIt does affect your work, mentally it feels like I don\u2019t want to live, everything ends.\u201d <\/em>(Mumbai)<\/p>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cI have to do it [transition] before death, at least for one day. That is how it is with us is that we just want to live for a day and die\u2026Well, they don\u2019t realize that we have not really lived our lives. Because we have not lived lives\u2026 on our terms.\u201d <\/em>(Mumbai)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Although mental health challenges were common in the community, participants reported <strong>limited use of mental health services <\/strong>due to a lack of queer- and trans- competent providers, mistreatment, and stigma associated with seeking mental healthcare. Previous experiences with mental health professionals often involved attempts to change the trans person\u2019s gender identity (to make them cisgender or non-trans) at the urging of parents.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cThe psychiatrist and counselors, most of them, like 80% to 90%, will always talk in favor of our parents. They will never talk in our favor. In a very rare case the psychiatrist or the counselor will say, \u2018No your son is alright.\u2019 Otherwise, in every other case they will always say, \u201cThere is some or the other deficiency in your son and we will cure that. You come for 10 sessions, you come for 4 months, come for 6 months, it will cost so and so&#8212;\u201d <\/em>(Mumbai)<\/p>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cI know a lot of friends who were taken by their parents to the psychiatrist and the psychiatrist said, \u201cYes this is a disorder and we will cure it.\u201d This is where the parents start pressurizing the kids and like in our brother\u2019s case, he mentioned that the doctor took it personally to cure him.\u201d <\/em>(Mumbai)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>&nbsp;Many cited the <strong>community as a great source of support<\/strong> and strength.<\/li>\n<\/ul>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-coblocks-accordion-item\"><details><summary class=\"wp-block-coblocks-accordion-item__title has-background has-pale-pink-background-color has-text-color has-black-color\">Education, Employment, and Public Spaces<\/summary><div class=\"wp-block-coblocks-accordion-item__content\" style=\"border-color:#f78da7\">\n<ul class=\"wp-block-list\">\n<li>Participants spoke of issues in <strong>education<\/strong>, including how lack of access to care, poor mental health and discrimination lead to poor performance and high drop out rates in school, especially for trans youth.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cPeople are facing violence, but they are not able to understand they are experiencing violence. And they are dropping out of school but not understanding that this is the result of violence.\u201d <\/em>(Mumbai)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Similar factors, as well as <strong>employment discrimination<\/strong>, make it difficult for transmasculine people to find jobs, stay at jobs, and feel safe living as a trans person in their workplace.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cIf you want to get an education, that\u2019s difficult, getting a job is also difficult. The government says, \u2018you can work anywhere\u2019. But it is difficult because there is the private sector, the government sector, the government people say, \u2018work in this environment if you want to, if not, leave\u2019.\u201d <\/em>(Mumbai)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Violence, harassment, and fear of <strong>experiencing violence and harassment <\/strong>are issues when navigating public spaces, including transportation and restrooms in public, at work, or at school.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cIn Delhi I was riding my bike, so like, a traffic policeman stopped me, and he asked me to show the documents, right? So at that time I hadn\u2019t gotten my documents changed, and he was from Haryana, so he\u2019s trying to say to me, [mockingly] \u201cCome on man, this document is a girl\u2019s! You\u2019re a boy! Where\u2019s the girl gone? Where\u2019d she go? Whose bike have you stolen?\u201d So basically saying that I\u2019m a thief.\u201d <\/em>(Mumbai)<\/p>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cWhat we have to understand is that a lot of people are getting killed because they are trans men. Because they don\u2019t, they can\u2019t access information to prove what they are to their parents or to their friends and family and so on and so forth. So a lot of them are getting killed. A lot of them cannot express themselves, so they suppress it\u201d <\/em>(Bengaluru)<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-coblocks-accordion-item\"><details><summary class=\"wp-block-coblocks-accordion-item__title has-background has-light-green-cyan-background-color has-text-color has-black-color\">Legal Gender Affirmations and Protections<\/summary><div class=\"wp-block-coblocks-accordion-item__content\" style=\"border-color:#7bdcb5\">\n<ul class=\"wp-block-list\">\n<li>Participants spoke of safety issues around being in a separate \u201ctransgender\u201d category legally, and feeling like this set them up for discrimination and violence.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cBecause if you\u2019re arguing from the point of legal gender recognition, then you can argue that this legal gender recognition is about your social safety. Tomorrow if my passport has TG on it, and I am living as a male person in society, then anyone seeing my passport might discriminate against me. Meaning this which was created for my protection, is being used to control me.\u201d <\/em>(Mumbai)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Some participants had <strong>difficulty changing legal documentation <\/strong>such as ID cards, to match correct gender identity and name.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cFrom transition, everything, what documents we legally get changed, like the Aadhar card, PAN card, voter ID, passport, everything. So I think there is a procedure, but it is very lengthy and one loses a lot of time in that.\u201d <\/em>(Mumbai)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Participants did not feel protected against discrimination, violence, or harassment by current anti-discrimination laws.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cIf something happens with trans people, they don&#8217;t take any steps for that. So there should be proper rights and laws for that also. If something happens with us.\u201d <\/em>(Delhi)<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-coblocks-accordion-item\"><details><summary class=\"wp-block-coblocks-accordion-item__title has-background has-pale-cyan-blue-background-color has-text-color has-black-color\">Family and Social Acceptance<\/summary><div class=\"wp-block-coblocks-accordion-item__content\" style=\"border-color:#f78da7\">\n<ul class=\"wp-block-list\">\n<li>Participants reported that many in the community are not supported in their identity by their families, with some being forced by family to participate in \u201ctreatments\u201d for their gender identity, including psychiatric, medical and alternative procedures.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cWhen my parents came to know about me&#8230;they took me to some psychiatrist right? Also I was hospitalized because of some of my relatives. So at that time, a psychiatrist asked me to wear a saree and marry the boy. I denied her and they trapped me in the hospital which was very difficult for me\u2026She started giving me female hormones.\u201d <\/em>(Delhi)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Participants described the <strong>familial and social pressure to live as a woman<\/strong>, marry a man, have children, and take on tasks traditionally for women, such as cooking and cleaning.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cThey [transmasculine people] are often pressurized to have sex, physical relationship and contact with a male. They are often asked, \u201dWhy aren\u2019t you interested in males? Is it because you have not tried it yet, at least give it a try once.\u201d Sometimes their own brothers \u2019try\u2019 or get someone else from outside to \u2018try\u2019. This is what happens to [pre-transition] men. Sometimes they are married to someone against their will, so there is this physical violence too.\u201d <\/em>(Mumbai)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Participants emphasized the <strong>importance of family support.<\/strong><\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cFamily plays an important role in this whole thing. When family accepts us, everybody else will also accept us.\u201d <\/em>(Delhi)<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-coblocks-accordion-item\"><details><summary class=\"wp-block-coblocks-accordion-item__title has-background has-pale-pink-background-color has-text-color has-black-color\">Priorities and Approaches for Transmasculine Health Research<\/summary><div class=\"wp-block-coblocks-accordion-item__content\" style=\"border-color:#f78da7\">\n<ul class=\"wp-block-list\">\n<li><strong>Protecting anonymity and safety of trans masculine people <\/strong>participating in research<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cWe know that we have to maintain each other\u2019s safety and privacy.\u201d <\/em>(Mumbai)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Online surveys in multiple languages, as well as private one-to-one interviews<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cOne-on-one in-person survey because that will make them more comfortable, we can talk to them and maybe they can do it more nicely and efficiently.\u201d <\/em>(Delhi)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>&nbsp;<strong>Sharing the results widely<\/strong>, including to families, schools, employers, policymakers, and healthcare workers<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cIf there is data on how people are &#8230; managing even being trans men, managing their life. That will be some good data to show to the parents. Like, there are people who are having jobs and managing their life, that might give them confidence that people are doing it.\u201d <\/em>(Delhi)<\/p>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><em>\u201cBasically we need awareness from the ground levels and villages and towns. The study was initiated in partnership with TWEET Foundation and Sampoorna Working Group, with preparatory work conducted between 2018 and 2021. The study topics and methods are based on what we learned from these community consultations held in 2019. In 2021, Sampoorna Working Group took the decision to withdraw from the project. Transmen Collective joined the study partnership in 2021.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<p class=\"has-black-color has-text-color has-background wp-block-paragraph\" style=\"background-color:#959595;font-size:16px\"><strong>What&#8217;s next?<\/strong><\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-background wp-block-paragraph\" style=\"background-color:#959595\"><em>Our Health Matters <\/em>has two phases. First, from June to August 2021, we conducted qualitative interviews in Hindi, Marathi, and English. Next, we will launch a multilingual survey in early 2022 to gather information from a larger number of trans men and transmasculine people.
